8. In MI1 in the scene with the giant monkey head and all the idols in front of it, one of the idols looks like a dog with a hat, and a rabbit on top of it... I'm guessing these are 'Sam & Max'...

No luck with finding another LucasArts logo in MI2 though :(

Disks 22, 36 and 114 which supposedly contain a part of the game where you enter a system of catacombs, but Guybrush says he'll skip that part...

Well, I just walked into the forest again, and when I look at the treestump it still asks for disks 22, 36, 114...

I thought maybe I copied it wrong but obviously I didn't... Have you double checked that 144?
